Meaning

"I can't thank you enough" is thanks turned all the way up. It means the help was so big that no amount of "thank you" feels like it covers it. You save it for real favors: someone drove you to the hospital, found your lost wallet, looked after your kids at short notice. For a held door, it sounds strange and too much.

It is sincere and works in any register, from a neighbor to a client email: "We can't thank you enough for your patience." People often add what they're thankful for with "for": "for everything", "for staying late". The usual replies are modest: "Don't mention it", "It was nothing", "Anytime". Saying "I know" back is a joke between close friends only.
