The pattern
"I don't know what to" plus a verb says you are stuck on a choice or a reaction. "I don't know what to say" is honest in a surprising or emotional moment. "I don't know what to do" asks, quietly, for help. The frame works with other question words too: "I don't know where to start", "I don't know who to ask", "I don't know how to explain".
Few sentences in English are this human. When someone gives you a gift or shares bad news, "I don't know what to say" followed by something simple, "Thank you" or "I'm so sorry", is often the right answer.
